ପ୍ରାରମ୍ଭିକ ପରାମର୍ଶ ପାଇଁ, କେବଳ ରୋଗୀଙ୍କ ସମ୍ପର୍କୀୟ, ସମସ୍ତ ପ୍ରାସଙ୍ଗିକ କାଗଜପତ୍ର ସହିତ ଡାକ୍ତର, ରେଫରାଲ ହସ୍ପିଟାଲ କାଗଜପତ୍ର ସହିତ ସାକ୍ଷାତ କରିବାକୁ ପଡିବ।ପ୍ରାରମ୍ଭିକ ପରାମର୍ଶ ପାଇଁ ରୋଗୀଙ୍କୁ ସିଧାସଳଖ ଆଣନ୍ତୁ ନାହିଁ।, ଆବଶ୍ୟକୀୟ କାଗଜପତ୍ର:-
ବାୟୋପ୍ସି ରିପୋର୍ଟ, ସିଟି ସ୍କାନ ରିପୋର୍ଟ, ଏକ୍ସ-ରେ ରିପୋର୍ଟ/ଅନ୍ୟାନ୍ୟ ସ୍କାନ ରିପୋର୍ଟ, ନିର୍ଦ୍ଦିଷ୍ଟ ଉନ୍ନତ ପର୍ଯ୍ୟାୟ କର୍କଟ ରୋଗ ନିର୍ଣ୍ଣୟ, ଡାକ୍ତର ରେଫରାଲ କାଗଜପତ୍ର, ହସ୍ପିଟାଲ ଡିସଚାର୍ଜ ସାରାଂଶ, ଆଧାର କାର୍ଡ, ସରକାରୀ ପରିଚୟ ପତ୍ର।

- 01 Advance information about the patient is necessary.
- 02 A referral form from the treating doctor detailing oncology treatment received, a brief medical history, and current medications.
- 03 Patients with advanced cancer who have completed active treatment or are not suitable for further curative treatment.
- 04 All patients with advanced-stage cancers experiencing distressing symptoms and in need of palliative care.
- 05 Patients on palliative care with a significant symptom burden requiring regular evaluation.
- 06 Patients and caregivers needing active nursing care, counseling, and bereavement support.
